Onyx M.D. Rewards FAQs:
- Who is eligible?
Most any Onyx M.D. Contract Healthcare Provider that has successfully completed an assignment is awarded points and is not participating in any other Onyx M.D. specialty programs.
- How do providers register?
Providers are automatically registered after the first time they earn points for any given month. The Onyx M.D. Rewards Support teams sends the providers a username and password.
- How are rewards points awarded?
Providers earn 1 Onyx M. D. Reward point for each dollar of compensation paid during a given month.
- When are points awarded or issued out?
Points are awarded 30 to 45 days after the month they are earned.
Example:
Jan 08, Dr. X was compensated $10,000 and earned 10,000 points. These points are issued no later than March 15, 2008
- How long can the points accumulate before they expire?
24 months from the time the first point was issued.
- When can providers place orders?
Providers can place orders online 24/7
- Why do Onyx M.D. contract providers need a username and password?
A username and password allows users to check points balances, change shipping address and check order status.
- Do Providers need a username and password to browse www.onyxmdrewards.com?
No, Providers are free to look and browse the site at anytime.
- Can the Provider buy something greater than the number of points they have earned?
No. At no time will Onyx M.D. offer the rewards points as a discount on a more expensive item that exceeds the Providers total current point value.
- How can Providers find out how many points they have?

- How does Onyx M.D. Rewards handle out of stock, color or size not available for items that are in the store?
The provider is contacted, informed about the situation, and asked to make a new choice.
- Is shipping included?
Yes. Free shipping is included on all onyxmdrewards.com items. We ship via USPS or UPS ground. No express shipping.
- How are items returned or exchanged?
All rewards gifts are final and may not be returned. After the first 30 days if an item is defective or damaged, we will attempt to work with the vendor where the item was procured. Provider will need to pay for return shipping. After 30 days, provider must work with the manufacture directly.
